Today, the whole nation is in shock as the launch of Chandrayaan 2 has been now called off by ISRO. The Launch of Chandrayaan-2 via GSLVMkIII-M1 was scheduled to launch on 15th July 2019 at 2:51 am but the Indian Space Research Organisation (ISRO) had to call off the mission 56 minutes 24 seconds before liftoff at 1:55 am

 

According to ISRO scientists, there were some technical snags in the launch vehicle system. As a result, the space agency had to stop the Chandrayaan-2 launch as a measure of abundant precaution. 

Here we see that though a lot of efforts were made during this dream project, there is still a lot of scope of improvement. And FMEA has the major role to play in figuring out and removing technical faults in any project.

But first, we need to understand the meaning of FMEA.

 

1. What is FMEA?

Failure Modes and Effects Analysis, abbreviated as FMEA, is a methodical strategy for tracing all the possible causes of failures in a project. This can help in prior detection of a possible failure.

The failure points that are found in a particular project of an organization are referred to as the failure modes.


3. What is Effects Analysis?

The potential impact of the Failure Modes is to be assessed on the basis of their seriousness, frequency, and complication. This process is called Effects Analysis. The detection failure modes and Effects Analysis should be done together systematically in order to avoid possible failure.

Yes, NASA has applied the FMEA strategy in almost all its space missions. Here are the instances:

  • In 1966, NASA had released its report about the use of FMEA procedure in its Apollo 11 moon landing mission, which was accomplished in 1969. FMEA was used to identify Single Point failures in the entire program. There were 420 such errors found during that mission!
  • Subsequently, NASA applied  FMEA on all other noteworthy space programs viz.:
  1. Vikings Mars landing mission (1975-78),
  2. Voyager (1977),
  3. Magellan Venus surface mapping (1989), and
  4. Galileo mission for studying Jupiter & its moons (1989).
